The first step in the TMS treatment process is determining whether or not TMS is an appropriate treatment for you. Shore TMS offers a free initial screening with our intake specialist to determine if you may be a candidate for treatment. During this consultation, we will review your symptoms, treatment history, and possible treatment plans. 

Shore TMS has a team in place to make the often overwhelming insurance process manageable. Our goal is to assist you in gaining access to TMS Therapy as quickly as possible by working with your insurance provider and securing coverage. Since insurance companies have their own requirements, you must first have a free consultation to determine if you meet those requirements.

If you are a possible candidate, after the initial free consultation, you will be scheduled to see Dr. Wolff, our director. Dr. Wolff will see you for a full psychiatric evaluation and discuss TMS treatment with you.

Initial Mapping Appointment

It is important to determine how to most effectively administer your individual treatment, your first session may last up to an hour and a half. At this first visit, Dr. Wolff will determine the appropriate placement and energy settings of the treatment coil.

Placement of the treatment coil and the strength of the magnetic field are important factors in successful TMS treatment. In order to do this, a test is done to determine what is called the motor threshold. A magnetic pulse will be repeatedly given to find the strength that is just enough to result in a movement of the fingers.


From there, the coil is moved forward to the left frontal lobe, specifically the Dorsal Lateral Prefrontal Cortex (DLPFC) and the energy levels are then adjusted.

Begin TMS Treatment

The usual treatment session lasts up to 37 minutes. As you sit reclined, comfortable, and alert, the TMS device delivers rapid MRI strength magnetic pulses in a 4-second sequence. The same 4-second sequence is repeated 2-3 times a minute. During the session you can watch television, listen to music, or simply sit back and relax.

Treatment sessions are then repeated by our expertly trained TMS Technicians each day using the settings initially determined by Dr. Wolff. A standard course of treatment involves sessions 5 days a week for 4-6 weeks, although this can vary depending on a patient’s response to treatment.


A tapering set of treatments is sometimes used to ensure your improvement. As you progress through your therapy, our Staff will regularly assess your progress and determine the eventual number of treatments. After the treatment, you can drive and resume your normal day. TMS is non-invasive, meaning it does not involve surgery, and also non-systemic, meaning no drugs are used, and therefore no drug side effects.

Is TMS For Me?


  • Am I a good candidate for TMS?

    a.            Suffer from Major Depressive Disorder


    b.            Have not received relief from multiple anti-depressant medications


    c.             Unable to tolerate anti-depressants or the side-effects caused by them


    d.            No active drug use or seizure disorder

  • What is the typical course of treatment? How long? Equipment used?

    a.            A total course of treatment is completed in about 6 weeks for a total of 36 treatments; including the initial mapping appointment.


    b.            Each treatment is approximately 37 minutes long; 5 days a week


    c.             Magstim TMS machine and coil. Each patient receives their own individual treatment cap that they will wear each session.

  • Is TMS Therapy safe?

    a. TMS therapy is an FDA-approved treatment for depression and has been known to be used in the treatment of many other neurological conditions. The treatment is always performed by a trained TMS technician and under the supervision of the prescribing psychiatrist. There have been very few risks associated with the treatment and it has been proven to be the least invasive treatment option for depression as well as having the smallest number of potential side-effects.

  • What are the side effects of TMS?

    a. Virtually none. Compared to other treatments for depression, rTMS is generally well tolerated. The most common side effects reported by patients are mild discomfort at the treatment site and the possible development of a slight headache during the first week of treatment.


    b. The most serious risk of rTMS is seizures. However, the risk of a seizure is exceptionally low and a thorough check on a client’s medical history is conducted to rule-out this risk.

  • What is the difference between TMS and ECT?

    a. TMS is NOT shock therapy and uses strictly magnetic pulses NOT ELECTRIC. Anesthesia is not required- the patient is awake and alert the entire time and is able to drive themselves to and from treatment. TMS does not prove to cause any of the undesirable side-effects as ECT such as memory loss, dizziness, insomnia, gastric issues, etc.

  • Does TMS cause memory loss?

    a. No. In some cases, patients report an incline in their cognitive functioning throughout the course of their treatments.


  • How long do the TMS treatment results last?

    a. Remission rates vary per patient.


  • Will my insurance cover TMS Therapy?

    a. We currently participate in most commercial insurances including but not limited to Medicare, Aetna, Cigna, United Healthcare, and most Blue Cross Blue Shield plans. Our staff handles all insurance authorizations for you- you are strictly here for your well-being.

  • What should I expect my first visit?

    a. Our office currently offers Free Consultations as a way for those who are interested to come in, view the machine, and become acquainted with the treatment process. The appointment is about 15-30 minutes, during which you meet the treatment team, ask questions, go over any concerns one may have. You are also given some additional information regarding the treatment such as articles, brochures, etc.

  • What do I do once my TMS treatment is completed?

    a. After one completes the first 30 TMS sessions, the last six are tapered out through usually a two-week period. Our office offers additional maintenance sessions two months following the final day of treatment at a discounted rate for those who have completed their full course of treatment with us.

  • What clinical information is available on TMS?

    a. Many studies have been completed regarding the TMS treatment. Our office also provides various articles and information on the treatment at the time of the initial consultation.
